Very nice solution here for FBA user management. but i got 1 minor problem when using the membership request web part. The captcha image does not seem to work in internet explorer.
From your screenshots i noticed you didnt use IE. Have you tested this in IE?

You're right! All testing was done in Chrome. Very strange that this same code for SharePoint 2010 (.Net 2.0) worked fine for IE. I'll get this fixed shortly. In the meantime, you can turn off the captcha in the web part properties.
It was an issue with the content type. This has been fixed in version 1.3.3.
