Default group appearing multiple times

When i go to approve a newly added user. 'default group' appears multiple times like shown below. It should only appear once I have checked the request review list and it doesn't seem to have those duplicate columns. Any ideas what i could be doing wrong?

My guess is that you've made a mistake when modifying the membership review list schema.

Perhaps you've just copied the guid's instead of generating a new one for each fields.

Or maybe not adjusted the name of the resource for the column name.
