Password Recovery email has wrong url?

Just found the url in password recovery email is wrong.   Here is what is being sent out RequestAccess/Pages/ChangePassword.aspx  It should be RequestAccess/SitePages/ChangePassword.aspx 

Where and how do I add the missing "Site"?

The url can be changed from 'Site Settings' -> 'FBA Site Configuration'
