Users Removed When Membership Request Approved

I'm having an issue where the FBA user is being removed from the site when I approve a membership request in the membership review list. I am using a secondary site collection w/ anon access enabled to handle membership requests and I'm unable to use the Review Requests feature because the users are being removed once they are changed to Status = Approved.

Any ideas?

Just an update on this issue for anybody experiencing the same problem - the email template had been set to an invalid format, which was preventing the email from being sent.
